Chemotherapy and target therapy as neo-adjuvant approach for initially unresectable colorectal liver metastases

چکیده
Although surgery is the most effective treatment for liver metastases in colorectal cancer patients, only 15-20% of these patients are suitable for a radical surgical approach, and metastases recurrence may occur at follow up. In the last decade, the use of pre-operative chemotherapy in combination with new biological drugs has been introduced. We reviewed data of neo-adjuvant chemotherapy strategies aimed at increasing the resection rate of liver metastases in colorectal cancer patients who were initially considered unresectable.

Strategies to increase the resectability of liver metastases from colorectal cancer.
Liver resection can provide long-term survival and cure for patients with colorectal liver metastases but is feasible in only 15-25% of patients. In the last few years several major developments have contributed to increase this resectability rate. Neo-adjuvant chemotherapy can provide response rates as high as 50%, allowing surgery in about 10-15% of patients initially deemed unresectable. Pat...

OBJECTIVE The aim of this study is to determine the hospital's efficiency as regards neoadjuvant chemotherapy for the resection of initially unresectable metastases for patients with metastatic primary colorectal cancer and to describe the chemotherapy used. METHODS Descriptive, retrospective study of patients with colorectal cancer from 2004 to 2007. The percentage of resection for metastase...
متن کاملOptimizing the approach to patients with potentially resectable liver metastases from colorectal cancer.
Liver metastases are a common event in colorectal carcinoma. Significant advances have been made in managing these patients in the last decade, including improvements in staging and surgical techniques, an increasing armamentarium of chemotherapeutics and multiple local ablative techniques. While combination chemotherapy significantly improves median patient survival, surgical resection provide...
متن کاملDefining the Role of Hepatic Arterial Infusion Chemotherapy in Metastatic Colorectal Cancer
Drs. Whisenant and Venook have provided us with a summary of the role of hepatic arterial infusion (HAI) chemotherapy for patients with metastatic colorectal cancer. In their abstract they state that there is confusion about whether HAI for unresectable liver metastases is useful, because European studies have been negative; they go on to say that the work in adjuvant therapy has mixed results....
متن کاملCurrent preventive treatment for recurrence after curative hepatectomy for liver metastases of colorectal carcinoma: a literature review of randomized control trials.
To review the preventive approaches for recurrence after curative resection of hepatic metastases from colorectal carcinoma, we have summarized all available publications reporting randomized control trials (RCTs) covered in PubMed. The treatment approaches presented above include adjuvant intrahepatic arterial infusion chemotherapy, systemic chemotherapy, neoadjuvant chemotherapy, and immunoth...
